We are always delighted to collaborate with our brilliant partners. We recently joined forces with colleagues from Novartis UK and NHS Nottingham & Nottinghamshire CCG on an article for the Integrated Care Journal about Population Health Management (PHM).

Published on 10 May 2021, it looks at how a PHM approach led by primary care will transform patient outcomes throughout the UK, enabling healthcare professionals to identify key groups at risk, proactively intervene and deliver the best value for their local populations.

We were delighted to contribute to the article, and thank Novartis Uk And colleagues Maria Principe, Mike O’Neil and Jack Rodber from NHS Nottingham & Nottinghamshire CCG for the opportunity.
