PULSE PCN insights Winter 2021

In April 2021 PULSE magazine launched PULSE PCN as a the new hub containing information for primary care network clinical directors.

They’ve just interviewed Andi Orlowski for the Winter 2021 issue of their magazine. In the latest issue Andi discusses the benefits of a Population Health Management (PHM) approach and how Primary Care Networks (PCNs) are critical for successful deployment.

PHM ensures the right care is given at the right time by the right person and allows primary care a greater opportunity to partner with other organisations to help address the health of the population.

You can read the full article in the latest issue, available on the PULSE website.

Andi was only allowed to provide one page on PHM for PULSE. However, he and our team are extremely passionate about PHM and have a lot more to say on the topic. If you’d like to find out more about how we could help you with PHM then please do get in touch.
